Answer the following questions:

— Do you snore?

— Do you often feel tired during the daytime?

— Has anyone observed you "stop breathing" during your sleep?

— Do you have or are your being treated for high blood pressure?

— Is your BMI greater than 30? (See box at right)

— Are you over 50 years old?

— If you are a male, is your neck size more than 17"? If you are a female, is your neck size more than 16"?

— Are you a male?

High risk: Yes to 3 or more
Low risk: Yes to 1 or 2
