To help patients achieve their fullest potentials once they leave the hospital, St. Anthony's Acute Rehabilitation Unit offers post-discharge follow-up programs, including:
- Stroke club
Offered twice each month at St. Anthony’s, this member-driven club enables stroke survivors and caregivers to discuss experiences and challenges.
- Stroke panel
New stroke survivors can ask questions and discuss concerns with experienced stroke survivors during these twice monthly sessions held on the Acute Rehab Unit.
This prosthetic clinic, offered on the Acute Rehab Unit, provides information, assistance, prosthetic recommendations and support to amputees.
- Amputee support group
Education and socialization are provided for amputees and their families, friends and caregivers.
- Orthotic clinic
People with orthotics needs (i.e., ankle-foot brace) receive follow-up care, orthotic recommendations and assessments during these clinics held on the Acute Rehab Unit.
- Home care service
Staff can help patients and families arrange for post-discharge in-home care.
