The majority of medication errors occur when patients are transferred, either from one hospital unit to another or from hospital to home.

At St. Anthony’s Medical Center, a patient’s complete list of medications is double-checked by the entire care team every time a patient is moved to verify type and dosage of every medicine. This ensures patient records reflect the correct type and dosage of each medication to prevent omissions, duplications and dangerous drug interactions.
