Scheduling Appointments
Call 314-525-4508 for information or to arrange an appointment. We offer early and late times to fit your schedule as well as a variety of group classes or individual appointments. Two to four visits are recommended initially. An annual follow-up visit is encouraged if you have diabetes.
New Patients
Save time in the office by completing your new patient paperwork at home. Patients with diabetes or pre-diabetes should download and complete Forms A and B. Patients seeing the dietitian for other nutrition concerns, including weight management, should download and complete Form C.
When you visit us, also please bring the following to each visit (click on links for a form that you may complete and bring with you):
- list of current medications
- glucose meter and records, if available
- any lab work or information from your physician
- food records
Annual Diabetes Follow-up
The Missouri Consensus Management Guideline for Adults recommends annual follow-up for best diabetes care. After your initial education, we offer many opportunities to obtain the information and motivation you need to maintain control of your diabetes and health. If it's been a while since you've seen us, we encourage you to join one of our "refresher" classes or come back for a follow-up visit. Call 314-525-4508 to set up your appointment or sign up for class. Please bring the following to your visit:

Perinatal Center
Classes for gestational diabetes are taught in conjunction with the nurses in the Perinatal Center. If you are scheduled for class, please complete the nutrition history form to help the dietitian better assess your (and your baby's) nutritional needs. If you have completed class and will be coming for your follow-up, please download and complete these forms to bring to your visit:
- glucose log (if not provided by the Perinatal Center)
- food diary
Insurance & Payment
Most insurance companies, including Medicare, cover much of the cost of diabetes self-management training and medical nutrition therapy. Please call the member services number listed on your insurance card to verify coverage. Ask about coverage for diabetes self-management training and medical nutrition therapy. If your insurance company will not cover your visit, please call us. Discounts may be available and St. Anthony's offers a payment plan to assist you.
